First-report-only, enabled, no outstanding run, resolved model key required (the key for the scanner's provider, or the default key when the scanner has none). Empty body or {} only. 409 first_report_already_exists/run_in_progress/scout_paused/no_model_key. 422 model_provider_mismatch. Deterministic empty-input reports do not invoke provider: No new observations or No observations in window; Nothing notable requires provider analysis.
